For a Counseling Session by Phone:
Please call (336) 896-1191 or toll free (888) 474-8015 to book an appointment. An information packet can be mailed to you or you may download, print and complete the information packet below prior to your counseling session.
Submit documents with check or money order for $30 payable to FInancial Pathways of the Piedmont.* Submit a copy of your driver's license for identity verification. When we receive your forms and payment, we will call you to schedule a telephone or in person counseling session, whichever you prefer. If you desire to get a quote on a Debt Management Plan (DMP), you must include copies of your most recent credit card statements.
*We will consider waiving the fee for hardship. If you believe you qualify, please include a hardship letter.
